IPSG 2: Improve Effective Communication Goal: Ensure that important patient information is communicated accurately, completely, clearly, and in a timely manner among healthcare professionals. Communication failures are one of the leading causes of medical errors. IPSG 2 focuses on preventing mistakes caused by misunderstandings, incomplete information, or delayed communication. Key JCI Requirements 1. Verbal and Telephone Orders When a physician gives an order verbally or by telephone: The receiver writes down the order. The receiver reads the order back to the physician. The physician confirms the accuracy. This process is called "Read-Back" or "Repeat-Back." Example: Doctor: "Give Paracetamol 1 gram IV every 6 hours." Nurse: Writes the order.
